Experts Warn Rollback of Hepatitis B Guidance Could Weaken Disease Prevention in the U.S.

Public health experts are raising alarms after U.S. vaccine advisers moved to scale back long-standing recommendations for hepatitis B immunization in newborns, a decision they say could reverse decades of progress in preventing a serious and potentially life-threatening infection.

For more than 30 years, U.S. health authorities have advised that all infants receive a hepatitis B vaccine shortly after birth, followed by additional doses in early childhood. This universal approach, introduced in the early 1990s, dramatically reduced transmission of the virus, which can cause chronic liver disease, cirrhosis, and liver cancer. Federal health data show the policy has prevented countless infections and saved lives.

That strategy is now being reconsidered. A federal advisory committee appointed by Health Secretary Robert F. Kennedy Jr. recently voted to narrow the scope of the recommendation. Under the revised guidance, the birth dose of the hepatitis B vaccine would be reserved for babies born to mothers who test positive for the virus or whose infection status is unknown at the time of delivery.

For infants whose mothers test negative, the panel suggested that vaccination decisions should be left to discussions between parents and physicians, including the option of delaying or declining the vaccine altogether. Critics argue that this shift introduces unnecessary risk and confusion into a policy that has long been praised for its simplicity and effectiveness.

Medical professionals warn that relying solely on maternal testing may leave gaps in protection. Testing errors, incomplete prenatal care, or changes in a mother’s infection status could result in infants being exposed without timely protection. Universal vaccination at birth was designed to account for such uncertainties, ensuring broad immunity regardless of individual circumstances.

Beyond hepatitis B, experts are concerned about the broader implications of the decision. They fear it could set a precedent for rolling back other evidence-based vaccine policies, potentially undermining public confidence in immunization programs and weakening the nation’s overall disease prevention infrastructure.

Hepatitis B is often transmitted at birth or during early childhood, and infections acquired at a young age are far more likely to become chronic. Public health specialists emphasize that early vaccination is one of the most reliable tools for preventing lifelong illness and reducing long-term healthcare costs.

As debate continues, doctors and researchers are urging policymakers to consider the full weight of scientific evidence and decades of real-world success behind universal infant vaccination. They argue that weakening proven prevention strategies could expose more children to avoidable harm and complicate efforts to control infectious diseases in the future.
